What Is Support at Home?
The Australian Government introduced the Support at Home program as part of major aged care reforms, replacing the former Home Care Packages Program and Short-Term Restorative Care from 1 November 2025.
Support at Home simplifies access to in-home aged care, focusing on choice, flexibility, and person-centred supports that adapt as needs change.
Collaboration with Trilogy Care – Package Management
Melbourne Care Services collaborates with **Trilogy Care** for Support at Home package management.
Under this collaborative arrangement, Trilogy Care provides package management and administrative oversight, while Melbourne Care Services focuses on the direct delivery of high-quality, in-home support services
Our Support at Home Services
We provide structured, goal-focused in-home support, including:
- Personal care and mobility support
- Meal preparation and household assistance
- Medication assistance and safety support
- Daily routines and wellbeing support
- Community access and social participation
- Skill development to support independence
Service Rates
Our standard Support at Home service rate (weekday, daytime) is:
- $70.23 per hour
- Care coordination is available and priced flexibly, depending on the level and type of services provided.
- Package-managed arrangements (in collaboration with Trilogy Care) 10%
Self-Managed (Consumer-Directed) Clients
Melbourne Care Services accepts and supports **self-managed (consumer-directed) clients**.
We work directly with consumers who choose to self-manage their Support at Home funding, providing flexible, transparent, and tailored services aligned with individual needs and preferences.
Our approach to self-managed clients includes:
- Clear service agreements and pricing
- Flexible scheduling and support options
- Collaborative communication with consumers and their nominated representatives
- Compliance with applicable guidelines and standards
Funding & Choice
Support at Home services may be accessed through:
- Package-managed arrangements (in collaboration with Trilogy Care)
- Self-managed (consumer-directed) arrangements
All services are delivered in accordance with individual care plans, funding arrangements, and relevant regulatory requirements.
